Certificate revocation requests may be made by the subscriber as a result of employment termination, forgotten password, or loss or compromise of the private key. Revocation requests may be made by the subscriber or by the Registration Authority (RA) as a result of employment termination or suspected compromise of the private key. All revocation requests, along with the reason for the request are sent to the IA for processing.
		Document the revocation request and the reason for the request.
		Sign and date the revocation request.
		Mail or fax the signed request to one of the ORC branches below:

		File the revocation request and store in a locked safe for a period of one year.
